Immunization Clinic at Plaza for 7th & 8th Graders

Plaza Middle School, in partnership with the Platte County Health Department, will be hosting an immunization clinic at Plaza on April 30th. The clinic will provide all required and recommended immunizations for students entering 8th grade on April 30, 2020. (Incoming 7th graders can get these done early as well.)

 The vaccines being offered are:

* Meningococcal/Menactra (Meningitis) [Required]

*Tdap/Boostrix (Tetanus, Diphtheria, and Pertussis) [Required]

*Human Papillomavirus (HPV) [Recommended]

*Hepatitis A [Recommended]

 If you are interested in taking advantage of this service, please complete and return the Consent and Registration Form (link) and Student Screening Form (link) to Nurse Berkley no later than April 16th.  Please read the letter to parents (link) and the FAQ page (link) for more information about what else is required and who can qualify.
