Mandatory MSHSAA Forms & ImPACT Testing

Each student that participates in Athletics will need to turn in a Physical Evaluation form, filled out by the student's physician.  These forms are good for 2 years.  The Physical Evaluation forms can be found on the MSHSAA website: https://www.mshsaa.org/resources/PDF/2019-20%20New%20Pre-participation%20Physical%20Evaluation%20-%20PPE%20Part%201-English.pdf

ImPACT Testing 

If your student is interested in participating in a Fall sport (Cross Country, Football (8th grade only), or Volleyball) please make sure they have a current physical on file and have taken or plan to take our IMPACT test. The physical form and IMPACT information can be found on our Plaza website under the activities tab (Mandatory MSHSAA forms and IMPACT testing). Your student must have both of these things prior to the first day of school/practice/tryouts or if they are planning to participate in summer camps/trainings they will need to have it prior to then.

 

What is IMPACT Testing?

Athletes are required to have an IMPACT (concussion baseline test) on file prior to participating in athletics. The IMPACT test is good for 2 years. Please note that your athlete MUST be 12 years old to complete this test. If they are not yet 12, we will have another session at the beginning of the school year for them to attend.

 

 

IMPACT Testing Dates (this will be done during the school day)

May 6th starting at 7:30am (This has been completed!)

May 13th starting at 7:30am - Small Gym (This is over!)

May 17th starting at 2:30pm - Flex Space

*If your student is a virtual student, please have them email Ms. Benjamin at benjamina@parkhill.k12.mo.us to schedule a time to come in and test or to get on the list to test in the Fall.*

 

The test typically takes between 45 and 60 minutes. You are welcome to stay and wait for your student outside or they can call when they are finished to be picked up. They are required to have parent/guardian permission to complete the test. You can find the permission slips here. If your student is not yet 12, they will test at the beginning of next year.

 

Please remember that every athlete is required to have a physical on file prior to participating in athletics. These must be completed by a doctor.
