What is SWAY?

Sway is a platform that our athletic trainers can use for baseline testing, sideline assessments, and the return-to-play process through an ever-growing battery of balance, cognitive, and functional assessments.

This will be scheduled for ALL student athletes with the coaches and athletic trainers. Dates to complete SWAY testing will be given prior to each sports season.

There is required documentation and pre-work that must be completed before your athlete will be able to participate in the SWAY testing. Please complete this paperwork on your PRIVIT account.

SWAY Test Plan for Fall Sports:

Football: Our athletic trainers will attend football camp and work through testing with as many football athletes as possible. Camp is scheduled for July 29th-August 8th 9:30am-11:30am at Park Hill High School. This is for 8th grade students who are interested in playing football this Fall.

Volleyball and Cross Country: TBD

This coming school year, the Park Hill School District will be transitioning to Privit, a trusted platform for managing student-athlete health information.

With the implementation of Privit, parents and guardians will have convenient access to securely update and manage their child's health information online. This includes medical history, emergency contacts, physicals, SWAY concussion testing permission forms, and other required MSHSAA documents. By utilizing Privit we can ensure that our sports training staff and coaches have access to accurate and up-to-date medical information when needed.

To facilitate this transition, we are asking that all students who plan to participate in a MSHSAA-sponsored activity have a Privit account.

·         All students participating in a MSHSAA activity must create an account and complete the required forms. Click Here to Create a Privit Account. Detailed instructions and help with account registration can be found here.

·         Coaches and school staff will no longer collect paper copies of physicals. Instead, parents/guardians must create a Privit account and upload a picture or file of page 5 of the MSHSAA physical form. Click Here to Download the MSHSAA Physical Form.

·         Many students have a physical on file that is valid for two years. If your student currently has a valid physical on file with the Park Hill School District, you must still create a Privit account and complete the required forms. You will have the option to indicate that your student has a valid physical on file; school staff will then verify and upload the physical we have on file.

We believe that this transition to Privit will significantly improve the way we manage student health records. Thank you for your cooperation and understanding as we work to implement this positive change.

A physician must complete the sports physical prior to the first day of tryouts and/or practice. Students WILL NOT be able to participate without a current physical. Here is the form you will need for the physical. Please note that this is a new form, old forms should no longer be used. This physical will need to be uploaded onto PRIVIT once completed. We are no longer accepting paper copies of physicals.

Please complete the annual MSHSAA pre-participation documents located on PRIVIT. Please note that these are new forms and old forms should be discarded. You must have these forms completed in PRIVIT prior to the first day of practice/tryouts.

While we prefer your student ride to and from competitions with their team, we understand that many of you have other obligations and may need to arrive late or leave early. We have put together a Parent/Guardian Transportation Permission Form (PDF) for families who may need to transport their own student athlete to and from competitions. This form does have some guidelines that will need to be adhered to in order to be honored by the district:

1. The form must be filled out and returned to the coach/sponsor two school days prior to the planned activity / trip.
    
2. Multiple games / matches / meets can be included on a single sheet. However, the dates listed must coordinate with scheduled events. If the activity is rescheduled or changed for any reason, a new form must be submitted and approved.
    
3. The student athlete / parent will be expected to sign-in and sign-out with the coach at the events.
    
4. Coaches will keep a list of parents/guardians whom a student can be released to. This list is created with the information placed on the transportation form provided.
   Example: If the mother is listed on the sheet and the father comes to pick up that student, we can not release that student to that person, and they will need to be transported on the bus.
    
5. Coaches WILL NOT leave any student behind if a parent/guardian is not at the event yet (not even with permission). Nor will they delay team departure back from an event. In this instance, that student will be expected to ride the bus back to the school to be picked up.

This year, we will be utilizing HUDL which is a service that allows us to record and stream home games. Joining the teams is reserved for athletes and coaches.
